Person-centred care

The Montessori Method to Dementia and Aged Care - supports ‘person-centred care’ - a trademark of Diagrama's methodology.

Person-centred care is an approach that supports your loved on to regain control and pursue a better quality of life based upon their unique background, their strengths, hopes, dreams and aspirations.

It is a way of working which supports them to retain (and develop) knowledge, skills and confidence to more efficiently manage and make informed choices about their own care.

By taking a person-centred approach to care our residents feel valued, and benefit from individualised support that meets their specific needs, with a supportive physical environment and a social environment that enriches their experience and wellbeing.

Working with you as a team

We work with you and your loved one as equal partners in a multi-disciplinary team in planning, monitoring, developing your family member's care and goals.Your loved one receive a full needs assessment prior to arrival which helps form their care plan and sets out how their needs will be met. Care plans include information such as waking/sleeping times, favourites meals, hobbies etc.

Nursing care

Our professional and dedicated staff care for people with physical disabilities, those who are terminally ill, have high dependency needs, have become infirm and also those who suffer from dementia and mental health problems.

Nursing care is delivered using a method known as key nursing: each resident is allocated a nurse who will soon become a familiar face and will answer any queries or anxieties that you or your family member may have.

Fully trained registered nurse provide round-the-clock care and are always alert to changes in your health, mood or behaviour - to make sure your loved one receives the best care, when they need it most.

Palliative care

Our team is also currently working towards the Gold Standard in delivering excellence in palliative and End of Life care.

This means we will work with you and your loved one to understand their future wishes and priorities and tailor care for people nearing the end of life.

This enables you and your family to discuss and record your future health and care wishes so that they are known and respected when the time comes.
